Mesothelioma Prognosis

The prognosis and the life expectancy of the patient are dependent mainly on the stage of mesothelioma when it is diagnosed.  But with mesothelioma, unlike some other cancers, it is difficult to determine the developmental stage when the cancer is diagnosed. This is because the disease is quite rare and the initial symptoms are not easily diagnosable. As this deadly disease is diagnosed only at the advanced stage, the survival rate is not very high. The history of mesothelioma statistics indicate that 10% of the cases diagnosed for about 3 years and about 5% will live for about 5 years.  Still, if the disease is diagnosed early such as stage one and is treated effectively, then 50% will survive 2 years and 20% people will survive 5 years.

Stage one mesothelioma indicates that the cancer is localized and has not spread to other parts of the body. It is found only in one area of the body like in the case of pleural mesothelioma, it is only in one part of the lung. There are different methods to identify the stage of mesothelioma for its prognosis:

Mesothelioma Stage One – Butchart Staging System

This method is considered to be oldest system to discover the degree of development for pleural mesothelioma. The staging is done based on the size of the primary tumor. In Stage One, only one of the sides of the lung has cancer which indicates that one part of the pleural membrane is affected by cancer. It could be also one side of the diaphragm and one lung.

Mesothelioma Stage One – The TNM Staging System

This is a recent kind of staging system which stage the cancer depending on the size of a tumor, presence of cancer in the lymph nodes and its spreading to other parts of the body.  When using the TNM method, stage one mesothelioma is found in the left or right pleura and has spread to either the lung, the pericardium or the diaphragm on the same side of the chest. In TNM stage one mesothelioma, the cancer is not found in lymph nodes and has not metastasized to other body parts.

When the Brigham System is used to stage malignant mesothelioma, the doctors generally focus and grade the stage of the cancer based on the successful removal of the primary tumor with surgery. So, with this approach and method the stage one mesothelioma patient has a better prognosis, because the other stages may not follow this stage indicating that the cancer has not spread to other parts of the body and has been restricted to only one part of the body. This method also provides for better treatment options.

However, it is quite difficult to diagnose this disease at stage one use both methods because the symptoms of mesothelioma are not clearly identifiable like they are in Stages Two, Three or Four.  In Stage One, the patient can be treated by undergoing a surgery to remove the primary tumor and the nearing tissue followed by radiation therapy or chemotherapy to kill any remaining cancer cells.

CDC Data Shows Workers Exposed to Asbestos at risk of Mesothelioma Cancer

Data released by the Centers for Disease Control and Prevention (CDC) has shown that despite federal restrictions on the commercial use of asbestos, workers who were exposed to asbestos-containing products on the job decades ago may still be at risk of being diagnosed with mesothelioma as well as other asbestos-related diseases. History of Mesothelioma and Asbestos

Asbestos is a cancer-causing material that was commonly used for commercial, industrial and construction purposes before being restricted in the U.S. during the 1970s. However, because workers may take as many as 50 years after exposure to develop symptoms of an asbestos disease, many workers who worked with asbestos on the job are only now showing signs of mesothelioma, asbestosis, lung cancer or other related conditions.

Treatments for Pericardial Mesothelioma including Veglin

Pericardial Mesothelioma Introduction

Pericardial Mesothelioma is an extremely difficult cancer to treat and presently, all treatment methods used have a poor success rate.  The chances of a patient’s survival depend largely on how early and how aggressively the cancer is treated.  If pericardial mesothelioma is treated when it has fully matured and developed then the chances of survival are very slim and the prognosis for a patient could be only a couple of months.  The latency period of pericardial mesothelioma (20 – 50 years), and its symptoms which are ordinary of other more common diseases such as pneumonia, make it extremely difficult to diagnose and therefore difficult to treat in its early stages.  In this way, a lot of pericardial mesothelioma treatments do not aim to cure patients but instead focus on reducing the symptoms.

Before a patient is treated, a doctor will evaluate him or her, and afterwards decide which treatment method is most suitable for the patient’s situation.  The patient’s age, medical history, general well being and even weight are taken into account before the doctor will decide which of the three treatments are most appropriate.  These treatments are surgery, chemotherapy and radiation therapy.

Surgery

There are two different types of surgery that can be used one of which is called aggressive surgery.  This is the most dangerous of all the treatment methods available and only the fittest of people are deemed strong enough to undergo this.  Aggressive surgery consists of removing large chunks of cancerous cells from inside the patient.  This is extremely dangerous in the case of pericardial mesothelioma seeing as the cancerous cells are in extremely close proximity to vital organs such as the heart and lungs.  Extremely skilled surgeons are needed to perform this operation.

The second type of surgery is palliative procedures and this type of surgery is used only for the purpose of reducing the symptoms of the cancer.  Palliative procedures are performed when pericardial mesothelioma is in its latest stages and is practically impossible to cure.

Chemotherapy

Chemotherapy is the most well known cure of cancers and involves using drugs to kill of cancerous cells.  However, many of the drugs used do not have a high success rate although researchers are discovering new combinations of drugs that work more effectively.  Many pharmaceutical companies are also endeavouring to find new drugs.  Researchers predict that they will soon discover a drug with a significantly improved success rate.

Radiation Therapy

This form of treatment uses doses of radiation to kill cancerous cells.  However, in the case of pericardial mesothelioma this is extremely difficult because as well as damaging cancerous cells, the radiation also damages vital organs such as the heart and lungs.  This makes using radiation therapy most effectively extremely difficult.  The dose of radiation is usually too weak to make a significant impact on curing pericardial mesothelioma.

Dual Therapy

Dual therapy is just a combination of surgery, chemotherapy and radiation therapy.  This consists of using surgery to remove the majority of the cancer cells and then using chemotherapy and radiation therapy to kill off any remaining cells.  This has proved to be an effective treatment method however also very dangerous.  Only the fittest young people are able to undergo this treatment.

Non-Chemotherapy Veglin Drug: Clinical Trials

Veglin is an anti-angiogenesis non-chemotherapy drug (angiogenesis inhibitor) that was developed by Dr. Parkash S. Gill, Oncologist and Lead Contributor for the Mesothelioma Research Foundation of America, for the treatment of a variety of malignancies including mesothelioma. Veglin is one of several newly developed non-chemotherapy drugs being tested for possible utilization in the ongoing struggle to combat malignant mesothelioma.

About Veglin as an Angiogenesis Drug

Angiogenesis refers to the physiological process by which new blood vessels are formed from existing blood vessels. Angiogenesis is a natural process of cellular growth and development; however, it also fuels tumor metastasis (spread and growth of cancer). Tumors have a limited capacity to grow unaided. In order to continue to grow and metastasize, tumors require a continuous flow of oxygen and essential nutrients. In order for tumors to receive such a continuous flow, they require blood vessel growth into the cancerous mass. Tumors induce the required blood vessel growth by secreting a variety of growth factors (VEGF). What Mesothelioma Treatments Are Available?

Currently, mesothelioma treatments include chemotherapy treatments, radiation treatments, surgical treatments and experimental therapies and drugs. Research news reports the limited success of present treatments for Mesothelioma.

Mesothelioma research news indicates that traditional treatments viz. single or multi-drug chemotherapy has a response rate of about 20%. Radiation treatments, usually used in conjunction with surgery or chemotherapy treatments can relieve pain and shrink tumors, making breathing easier. However, radiation treatments can also cause severe side effects.

Efforts To Treat Mesothelioma Cancer

Mesothelioma can take between ten and sixty years to develop after inhalation of respirable asbestos fibers, but Mesothelioma progresses fast. Radical surgeries as mesothelioma treatments have had limited success. Surgical treatments attempted include extra-pleural pneumonectomy (EPP) and pleurectomy.

Recent news about mesothelioma treatments  indicates that in EPP over 15% patients die soon after surgery. EPP involves removal of pleura, diaphragm, pericardium, part of the phrenic nerve and the entire tumor-affected lung and has to be supported by chemotherapy or radiation.

The pleurectomy treatments involve removal of the pleura without removing the whole lung. These treatments are best in patients with good lung expansion who tend to accumulate fluid. In combination with radiation pleurectomy is reported in treatment news to have given 22-month survival in 50% and 24-month survival in 41% cases in a 27-patient group with mesothelioma of the epithelial subtype.

Surgical treatments of mesothelioma have not been shown to offer significant advantages over the limited non-surgical treatments. Experimental drugs under evaluation include treatments by Onconase, Lovastatin and an Endostatin and angiostatin combination.

What Can I Do When My Boyfriend has Mesothelioma and is Jobless?

After 3 happy years together, my jobless boyfriend has been diagnosed with Pericardial Mesothelioma. His health had been great until recently when he started having trouble breathing and shooting abdominal pain. Initially I was shattered and I was not sure how to deal with the news. Then I realized that it is important to consider what to do next to help improve his health condition. I had heard that some courses of treatment would make it possible for him to get over the disease and he might get completely cured if the disease is in its initial stages. I was also sure that he would not consider treatment unless he was fully aware of the disease and knew that the treatment would not cost him money and he has been jobless for over a year.

We later took a trip to visit a lawyer who deals with this disease diagnosis because the cause is usually employer negligence. He said we need to be prepared mentally about the lawsuit and I have to make him sure that I can be there to support my boyfriend in any situation because this lawsuit can be a long, tough road. While the lawyer said there is little I can do as a non-official family member, being there to support him is the most important thing I can do.

Fortunately, his finances don’t play a role in the lawsuit against his former employer because the lawyer only gets paid if we win. In preparing for the case, we have to look for a capable healthcare provider who can testify on his behalf. In the case of Peritoneal Mesothelioma, there can even be abdominal swelling, weight loss and accumulation of fluid in abdominal cavity. Apart from these, blood clotting abnormalities, anemia and fever are also found commonly. In the case of pleural Mesothelioma, blood in the sputum, chest wall pain, anemia, fatigue and pleural effusion are also common. In severe cases, the person may develop many tumor masses or pneumothorax (collapse of the lung).

Diagnosis of Mesothelioma

Mesothelioma does not affect the bone, brain or adrenal glands. Pleural tumors affect only one side of the lungs. The first process of diagnosis is to check the patient’s history for any asbestos exposure. Various tests like CT scan, chest x-ray, lung function tests, MRI scan needs to undertaken to diagnoses the disease. Treatment of Mesothelioma using conventional therapies like radiation, surgery hasn’t been very successful and new treatments are being developed. Unfortunately, the survival rate beyond one year after diagnosis is incredibly low.

A recent report from the U.S. Environmental Protection Agency about its own apparent mishandling of asbestos in demolition projects underscores the pervasive threat this cancer-causing material poses to Americans health.

If the national government department in charge with managing the public’s safety  from environmental hazards fails to protect themselves and their contractors from asbestos exposure, then it becomes a serious concern for everyone about the dangers faced by employees of less-conscientious organizations.

This bad news was recently released in a December 14, 2012  memo from the EPAs inspector general which emphasized concerns about the use of unapproved demolition methods at EPA sites where asbestos was present.

Because inhaling asbestos fibers is known to cause lethal cancers such as mesothelioma, demolition crews are required by law to follow strict protocols to prevent exposure to the deadly material.

The EPA report indicated that unapproved asbestos-handling methods were in use at several sites, including one in Washington state and another in Kentucky. The inspector general also cited videos and photos that showed unprotected government workers and contractors present during experiments with unapproved alternative asbestos control methods in Arkansas and Texas.

Current evidence demonstrates that many people  developed mesothelioma cancer or other asbestos-related diseases after being exposed at work to toxic asbestos decades ago.  And now it is very alarming to discover that workers continue to face exposure to this cancer-causing material even at job sites that are under the control of the government’s environmental watchdog.

Asbestos, which was once extensively used in a wide variety of applications, is the only known cause of mesothelioma. The disease is a rare but very aggressive form of cancer that usually affects the lining of the lungs. There is no known cure. Many victims do not develop mesothelioma until several decades after their exposure.

Because asbestos products remain in many older structures, the EPA has developed specific procedures for safer handling of asbestos during demolitions and renovations. The standards require specialists to remove asbestos-containing materials while minimizing the amount of asbestos dust produced.

Because the standards apparently were not followed at several EPA sites, according to the inspector general, the agency should notify workers and the public in the surrounding areas of the possibility that they were exposed to asbestos.

Modern workers who renovate older buildings are also at risk for mesothelioma inducing asbestos exposure.

The historic study is usually followed by thorough physical examinations including chest X-rays, lung function tests and CT scans or MRIs.

There are few treatments  for mesothelioma that can provide some sort of cure by themselves, and surgery has proven frequently ineffective on mesothelioma.   Chemotherapy is the most commonly administered effective therapy, and immunotherapy treatments have provided various results, some of which were in fact successful.  There are clinical trials of new treatments that are in Phase II and Phase III study that are proving to be very hopeful of a cure to mesothelioma cancer.
